To keep track of your school outreach CE points, complete and print out the form below or download the School Outreach CE Points form as a PDF file.

You'll want to save your completed form with your other continuing education documents. You may be asked to provide a copy of this form when you submit your recertification paperwork to ATA at the end of each three-year reporting period.

Number of continuing education points earned: (Each full hour of school outreach presentation time earns two ATA CE points, subject to a maximum of six CE points in each three-year reporting period. Two or more shorter presentations may be combined to reach the one-hour mark.)   Save this completed form with your other continuing education documents. You may be asked to provide a copy of this form when you submit your recertification paperwork to ATA at the end of each three-year reporting period.
